Pradhan Mantri Jan Arogya Yojana (PM-JAY) is implemented to reduce the financial burden on poor and vulnerable groups arising out of catastrophic hospital episodes and ensure their access to quality health services was conceived.
STEPS TO FOLLOW

The beneficiaries need to go to the District Civil Hospital and show the BPL card and if their name is in the Socio-Economic Caste Census, 2011 report they will get the card issued.

Those who are not in the census report they will have to submit the BPL card at the District Hospital and will get the card issued.

Once SECC number received, apply for Jan Arogya Yojana card through nearest Common Service Centre.
